Although the general mechanism of idiosyncratic drug-induced liver injury (iDILI) has been controversial, there is abundant evidence that most iDILI is mediated by the adaptive immune system. The histology of hepatocellular iDILI is virtually the same as that of viral hepatitis, and the major cell responsible for hepatocyte cell death is the cytotoxic CD8+ T cell. However, an innate immune response is required to initiate this adaptive immune response. There will be a discussion of these mechanisms, how they can be studied, and how they can be used to develop biomarkers to predict the risk that a drug candidate will be associated with a significant risk of iDILI.
